Community : Care plans
It is easy to set up as many care plans as you require. Each care plan can record goals, visiting frequency, action history and evaluation. The care plans are integrated into the patient record, and can be shared with other clinicians in your organisation. The care plan template system makes it easy for you to change templates as care requirements change.
The Visit Management tool includes a Visit Scheduler, Monthly Planner and the ability to pick up all patients with planned visits according to their care plan. It’s easy to print off a visit list, filtered by staff member or caseload.
