SystmOne Social Services has been developed in conjunction with experts working in the Social Services and the NHS today. It is the first of its kind, linking both health and social care information.
The software supports both case management and administrative work processes as well as client management, creating a seamless client journey from the moment they are referred to social care to the moment care is delivered and throughout the continuation of the care process.
As use of SystmOne is widespread in primary care, we have the unique opportunity to achieve an integrated care record between social care and healthcare. As a result of this, both social care and health care professionals have the potential to see important details about a patient upon which decisions can be based. Better access to information results in better care.
Patient journey
A patient journey through a contact with social services might cover the following:
- Concerned person/health care professional contacts Social Services.
- Person is then referred into social care.
- Social worker will then complete an assessment with the client to establish their needs, which can be input electronically into SystmOne.
- Staff will create a package of care to assign services and actions to meet the needs of the client.
- Staff will setup events for each service/action and this will produce the total cost of the package.
- The package is then authorised by management and the care is delivered based on the scheduled events. As part of the package of care, certain actions will be carried out by healthcare staff and a referral can be made to a healthcare organisation for treatment.
Record access
When a patient with an existing record on SystmOne consents, their details can be retrieved and pulled through into social services, thus reducing the risk of duplicate records between social care and healthcare and providing the benefit of a holistic record.
Workflow
Workflow is driven by SystmOne 'Tasks'. Each member of staff has their own 'To Do' list, making it easy to progress cases and perform actions.
Reporting
SystmOne allows Social Services to report on activity in the organisation. A built-in report provides all the information needed for the Social Services Key Performance Indicators.
In addition, users can set up specific reports to gather results on any aspect of the social care record, including service costs. Reports can be used to perform detailed analysis, for example, on budget code and services offered.
SystmOne's user-friendly reporting interface makes it simple to set up even the most complex reports. A comprehensive library of pre-defined reports is also supplied for users to build on. Accurate reports can be run at any time, without impacting on the rest of the organisation, and the results can be broken down for easy analysis.
